We are delighted to welcome you to our practice and are pleased that you chose us to serve your dental needs.
Please complete the following New Patient Forms and bring them, along with any dental insurance cards that might apply to you, to your first visit.

At your appointment we will take x-rays as part of your dental exam. To avoid a duplication of x-rays or the fees incurred to process them, please fill out and send the Authorization to Release Dental Records form to your prior dentist as soon as possible. If you prefer to call your prior dentist, please refer to the information on the form.
Authorization to Release Dental Records
Please note that full payment or insurance co payments and co insurance (your estimated portion) is due at every appointment as indicated in our Financial Agreement. Please read the Financial Agreement before your appointment as it is important that you have a clear understanding of our policies and procedures. If you have any questions we would be glad to assist you.
